  2. Who is the affected person? Name, age, and home address.
  3. What happened? Describe the reason for the call. What illness, injury, etc. occurred?
  4. Where is the affected person located? E.g. hotel, hospital, etc.
  5. Who is the contact person on-site? On-site contact person, e.g. attending doctor or police station.
